Make a concern specific and traceable.
Report suspected clinical, wording, accessibility or technical errors without sending patient information.Include
- Exact page URL and item title or number.
- The exact wording, value, dose, table cell or interaction that may be wrong.
- Why it may be wrong and a primary or authoritative source if available.
- Device/browser only when the problem is visual or technical.
Do not include
- Patient-identifiable information.
- Passwords, session details or complete access codes.
- Confidential interview or workplace documents you are not authorised to share.

Email the details with the subject ‘AFAQ correction’. Suspected safety-critical issues should say so clearly in the first line.
